I've been on several projects that blew up (in a bad way) at the end, and the PMs were surprised. But it was kinda predictable, to all of the devs, there just wasn't a great way to raise these vague concerns during the project. I built ProjectPoll so teams can communicate more honestly and track confidence more objectively.
